Permit Forms & Instructions

Permit forms for the City of South Miami Building Department are color coded for convenience of processing and record keeping. The forms, in Portable Document Format (PDF), are available online and should be readable by all types of computer. Some forms contain active fields that allow you to fill out the forms online and email them to the Building Department. Alternatively, they can be printed at your home or place of business and submitted in person to the Permit Clerks.

IMPORTANT: ONLY colored forms and originals of notarized forms will be accepted at the Permit Desk!
The completed color-coded forms can be downloaded and printed in color on your printer. Alternatively, you may request colored paper from the Building Department in person or via mail if you do not have access to a color printer.
